<p>This study aimed to explain the role of organizational culture and identify cultural mechanisms contributing to the prevention of employee mental health problems in complex work environments. This qualitative study employed an inductive thematic analysis approach. The participants were 24 employees, professionals, middle managers, and senior managers recruited from technology, financial, healthcare, industrial, and public service organizations in Tehran, Iran. Participants were selected through purposive maximum-variation sampling. Data were collected exclusively through individual semi-structured interviews lasting approximately 45–75 minutes. The interviews explored participants’ experiences of organizational values, behavioral norms, managerial practices, interpersonal relationships, workload pressures, responses to psychological distress, and organizational support systems. Theoretical saturation was reached during the twenty-first interview, after which three additional interviews were conducted to confirm the stability and completeness of the emerging categories. Interviews were transcribed verbatim, anonymized, coded, and analyzed using NVivo 14. Credibility and dependability were enhanced through participant checking, peer review, constant comparison, maintenance of an analytical audit trail, and documentation of coding decisions. Five main categories were identified: “supportive leadership and genuine prioritization of mental health,” “psychological safety and normalization of conversations about distress,” “organizational justice, transparency, and predictability,” “realistic workload regulation, flexibility, and protection of work–life boundaries,” and “social support, collective learning, and responsive employee-care systems.” Participants indicated that a preventive organizational culture develops when concern for mental health is reflected in routine decisions, resource allocation, managerial conduct, job design, performance expectations, and responses to employee difficulties rather than being confined to symbolic or short-term wellness programs. Organizational culture can function as an upstream preventive infrastructure by reducing stigma, facilitating employee voice and early help-seeking, balancing job demands and resources, strengthening organizational justice, and creating reliable pathways for support. In complex workplaces, sustainable prevention requires moving beyond individual resilience programs toward a systemic approach that embeds psychological health in leadership accountability, human resource policies, work design, communication practices, and organizational governance.</p>
