<p>This study aimed to explain how lifestyle patterns and health-related behaviors interact with psychological vulnerability among high-risk populations living in Tehran. This qualitative study was conducted using an inductive thematic analysis approach. The participants were 24 adults living in Tehran who were considered at increased social and health risk because of experiences such as substance use, housing instability, interpersonal violence, incarceration, or precarious employment. Participants were initially recruited through purposive maximum-variation sampling, followed by theoretical sampling until theoretical saturation was achieved. Data were collected exclusively through individual semi-structured interviews lasting between 45 and 78 minutes. With the participants’ permission, the interviews were audio-recorded, transcribed verbatim, anonymized, and analyzed using the six-phase thematic analysis process. NVivo software was used to organize transcripts, codes, analytic memos, categories, and relationships between themes. Member checking, peer review, maximum-variation sampling, reflexive memoing, and maintenance of an audit trail were used to enhance trustworthiness. Four main themes were identified: “erosion of everyday routines under conditions of instability,” “health behaviors as survival and emotion-regulation strategies,” “a mutually reinforcing cycle of psychological distress and unhealthy behavior,” and “the dual role of social relationships and service systems.” Participants described irregular sleep, poor-quality nutrition, physical inactivity, smoking, substance use, self-medication, and delayed healthcare seeking as responses to resource scarcity, unsafe environments, stigma, and psychological pressure. Although these behaviors provided short-term relief or helped participants cope with immediate threats, they contributed to fatigue, shame, reduced self-efficacy, anxiety, depressive symptoms, and further disengagement from healthcare. Social acceptance, nonjudgmental professional relationships, continuity of care, and small achievable lifestyle changes were described as protective factors. Lifestyle in high-risk populations cannot be interpreted solely as the outcome of personal preference or individual responsibility. It is shaped by the interaction of structural disadvantage, traumatic experiences, psychological distress, social relationships, and access to supportive services. Effective interventions should integrate lifestyle modification with trauma-informed psychological care, harm-reduction services, stigma reduction, social support, and practical assistance related to housing, nutrition, employment, and healthcare access.</p>
